Practice Specialist, Post Majority Support Services (Kîwîtin Acâhkos – Yidah)

Location: Edmonton, AB

The Post Majority Support Services program provides individualized support to First Nations young people aged 18 – 26 as they prepare for independence. The program is designed for youth exiting or leaving care due to having reached the age when mandated or legislated services end and will serve youth aging out of care and who are considered ordinarily resident on-reserve.

Treaty 8 Urban CFS office is hiring a Practice Specialist. This role will report to the Supervisor and will be based in Edmonton, Alberta. The Practice Specialist will be responsible to assess, evaluate, screen, and monitor clients of Post Majority Support Services. This role will manage case assignments, review and follow up, support, counsel and advocate to meet the client’s needs. They will help evaluate, develop individual plans, and collaborate and liaise with program and service providers to help and advocate for youth and young adults in the Post Majority Support Services program.

Responsibilities

• Responsible for facilitating the process of case management.
• Conduct initial and comprehensive assessments of identified needs and support systems, and develop individual plans.
• Actively engage and work with various internal and external resources identified by the needs of youth and young adults.
• Promote health and well-being through the lens of an Indigenous world view.
• Skilled collaboration and maintain professional relationships with program and service providers, Elders, community, government agencies, partners.
• Advocate for connecting Indigenous youth and young adults to their community, culture and teachings.
• Maintain case notes and document in case data management system as required.
• Maintain accurate and relevant client records as per legislation.
• Able to manage on-going case follow-up.
• Other duties as assigned.
• Will be required to be on-call after hours.
• Travel may be required.

Knowledge and Skills

• Ability to practice in an anti-oppressive manner.
• Ability to work in an environment that is culturally diverse.
• Critical thinking and problem-solving skills.
• Case management experience.
• Strong knowledge of community resources including but not limited to social, legal, health and financial (internal and external resources).
• Demonstrated experience with negotiation, trauma informed counselling, crisis management and problem-solving skills.
• Active understanding of colonialism, intergenerational trauma, the 60’s Scoop, the residential school system and its ongoing traumatic effects on Indigenous communities.

Qualifications

• Diploma or Degree in Human Services, Social Work or related field from an Accredited Education Institute.
• A minimum of four (4) years of relevant work experience.
• Satisfactory Criminal Record Check including vulnerable sector and Child Intervention (Welfare) Record Check (must be dated less than 3 months upon hiring).
• Valid class 5 driver’s license, reliable transportation with $2 million liability insurance
• Provide 3-year driver’s abstract.
• Strong knowledge and experience working with Data Base, Microsoft Office Suite, including Word, Excel, Power Point and Outlook.
• Knowledge and understanding of Treaty 8 First Nation communities, culture and language an asset.
