The Post Majority Support Services program (PMSS) provides individualized support to First Nations young people aged 18-26 as they prepare for independence. The program is designed for youth exiting or leaving care due to having reached the age when mandated or legislated services end and will serve youth aging out of care and who are considered ordinarily resident on-reserve.
Treaty 8 Urban CFS office is hiring a Practice Specialist for the PMSS program. This role will report to the Supervisor, PMSS and will be based in Edmonton, Alberta. The Practice Specialist will assess, do the initial intake, evaluate, screen, and monitor youth and young adults in the PMSS program. This role will also manage case assignments, review, and follow up, support, counsel, and advocate on behalf of the young adults in the program. The Practice Specialist will assist in the evaluation, development of individual plans, and collaborate and consult with program and service providers to help Indigenous youth and young adults transition towards independence and adulthood as part of the PMSS program.
Responsibilities
- Will be the initial contact for young adults accessing the PMSS program.
- Responsible to provide intake, assessments and make appropriate referrals for young adults.
- Create and record new files in the database management system with updated information and accurate notes.
- Will provide information to young adults such as income assistance, housing, health services, addiction and recovery, and education referrals with contact numbers.
- Responsible for facilitating and monitoring case files.
- Conduct initial and comprehensive assessments of needs, support systems, and development of individual plans.
- Actively engage and work with a variety of internal and external resources. These resources will provide a strong support network for Indigenous youth and young adults in the PMSS program.
- Collaborate and maintain professional relationships with program and service providers, Elders, communities, governments, and other agencies.
- Advocate, support and whenever possible, connect youth and young adults to their Indigenous culture and teachings.
- Shall perform on-call shifts in rotation and responsible for responding to incidents, handling emergencies and other duties relevant to the PMSS program.
- Promote health and well-being through the lens of an Indigenous world view.
- Maintain up-to-date case files with concise, accurate notes and documents in the data management system.
- Liaise, advocate, and refer youth and young adults to relevant services and programs.
- Manage on-going follow-up for youth and young adults in the PMSS program.
- Home visits must be conducted weekly and monthly – to ensure clients’ wellbeing needs are being met and assessing their home environments.
- Weekly and monthly supervision with direct supervisors to ensure case management knowledge sharing.
- Signs of Safety – mapping and trajectory of client’s case management must be adhered to for every client file.
- Contact notes must be on file – input weekly on data systems and on the hard file.
- Other duties as required.
Qualifications
- Diploma or Degree in Human Services, Social Work or related field from an Accredited Education Institute.
- A minimum of four (4) years of relevant work experience.
- Satisfactory Criminal Record Check including vulnerable sector and Child Intervention (Welfare) Record Check (must be dated less than 3 months upon hiring).
- Valid class 5 driver’s license, reliable transportation with $2 million liability insurance
- Provide 3-year driver’s abstract.
- Strong knowledge and experience working with Data Base, Microsoft Office Suite, including Word, Excel, Power Point and Outlook.
- Knowledge and understanding of Treaty 8 First Nation communities, culture and language an asset.
